Service Coordination is a person-centered service designed to provide primary support to waiver applicants/participants in accessing essential services, including Medicaid State Plan benefits, educational, vocational, social, medical, and other necessary services. These interventions aim to ensure the health and safety of the waiver participant while promoting increased independence, integration, and productivity within the community.
The Service Coordinator (SC) will work closely with applicants/participants to facilitate the acquisition and coordination of all services required to help them return to or remain in the community.
